Registration fee:  
  $800 per team if registered before June 15, 2009
  $850 per team if registered after June 15, 2009
   Minimum 50% deposit due at registration; balance due 15 days prior to start of tournament.

To register for the Summer Championships, take the following steps:

          Step 1:   Register to play in the Summer Championships.

                         Option 2:  Mail the official Entry Form with payment to:

                                              Summer Classic Registration

                         Option 3:  Fax the official Entry Form to (623) 773-8716.


         Step 2:  Make payment.
                       
                         Option 1:  Mail a check, payable to City of Peoria, to the address above.
                                      
                         Option 2:  Call (623) 773-8700 to pay with a credit card.
