INFORMATION

If you are not completely satisfied with the activities and programs offered, we will
assist you in the following ways:
TRANSFER you to another activity or class
immediately
CREDIT your account in the computer
immediately which then can be used by any family member for any recreation program in the
future
REFUND you money. If you paid by
credit or debit card, the amount will be credited to your account. There will be no
cash refunds. If you paid by check, refunds take approximately three to four weeks
to process and will be mailed to the address on the registration form. Refund checks
will be made payable to the head of the household unless otherwise directed.
Refunds will be given unconditionally and
will be processed form the date we receive them.
No refunds, credits or transfers will be
issued after the program or session has concluded.
Refunds will be given for registration fees
only. It is the discretion of the Recreation Supervisor or Recreation Coordinator
whether a refund will be given on a commodity (e.g. supplies, tickets, wearing apparel,
etc.) that has already been purchased for the participant.

NON-Resident Fees, Late Fees
Cancellations
Nothing cancels a program quicker than everyone waiting until the last minute to
register. There is a point when a decision must be made on whether a program will be
held or not. So remember, please do not wait until the day before the activity or
class begins to register, it may be too late. Programs that do not meet the minimum
enrollment will be canceled and a full refund will be issued or you may transfer to
another activity.

The City of Peoria endeavors to make all of its programs, services and facilities
accessible to, usable by and available to qualified individuals. Persons with

Youth Scholarship
Program
The Community Services Department is committed to making its programs accessible and
affordable to all citizens in Peoria. The Youth Scholarship Program enables the
Community Services Department to fulfill this commitment to the residents of Peoria.
You may qualify for all youth recreation program excluding DES funded programs such as
AM/PM program, Summer Camp and Little Learners (formerly Tot Time). Please call

Attention Active
Participants
By their very nature, many Recreation
programs involve body contact, substantial physical exertion, emotional stress and/or use
of equipment that represents a certain risk to users. It is recommended that you
check with your doctor prior to participating in Recreation activities. Registrants
in any program assume responsibility for any risk, implicit or direct, by participation in
said activity or facility.

softball fields and multipurpose fields that are available to rent for your private party,
