Provide a Cover page with the following: (As-Built certification statement, Blue stake note, Key map, Project name, address, location, legal description, notes section, etc). 2. Add a construction notes section to the Cover Page with the following notes: 1) The Contractor shall contact all utilities for Blue Stake locations prior to any construction activity. 2) Existing landscaping and / or irrigation system disturbed by construction shall be replaced in kind by the contractor. The Contractor shall call and coordinate this work with the City of Peoria's Community Development Services Landscape Maintenance Supervisor at (623) 773-7472. 3) The Contractor shall provide Traffic Control during construction per the City of Phoenix Traffic Barricade Manual and the latest Manual of Uniform Traffic Control Devices (MUTCD) approved by Arizona Department of Transportation (ADOT). 4) The Contractor shall provide an off duty Police Officer any time there is construction within 300 feet of a signalized intersection. 5) The Contractor shall obtain all necessary permits from the City of Peoria prior to construction. 3. Add a Signing and Striping Notes section to the cover page with the following notes: 1) The Contractor shall notify the City of Peoria's Engineering Inspection Division at least 48 hours in advance of any striping at (623) 773-8445. Striping completed prior to the City's inspection shall be removed if it is not consistent with City standards. 2) The obliteration of conflicting striping shall be accomplished by water blasting or other methods approved by the City. 3) Crosswalk, Stop Bar, and Turn arrow pavement marking installations shall be done in accordance with the City of Peoria's standard, Alkyd Thermoplastic material applied at 90 Mil thickness. Longitudinal striping shall be installed with water based striping paint as per the signing and striping plan. 4) Signing shall be installed as per the City of Peoria details # 132-1 and 132-2. 5) All signs are to be ASTM Type IV High Intensity Sheeting. 6) All signs are to be provided and installed by the developer per the approved plans. 7) The contractor shall return all removed traffic signal equipment / signs to the City of Peoria Municipal Operations Center, 8850 North 79th Avenue. Contact the Traffic Maintenance Supervisor at (623) 773-7470. 8) Signing and Striping installations shall be in accordance with the current edition of the Manual on Uniform Traffic Control Devices (MUTCD) adopted by the Arizona Department of Transportation (ADOT). 9) Unless otherwise noted, all dimensions are to the face of curb and the center of the stripe. In the case of a double stripe, dimension is to the center of the double stripe. 10) If the approved plans do not match existing field conditions, the City Inspector may make changes they deem necessary. 4. Provide an approval signature block on the cover page to read as follows: RELEASED FOR PERMITTING: This set of plans has been reviewed for compliance with City requirements prior to issuance of construction permits. The City neither accepts nor assumes any liability for errors or omissions. This compliance approval shall not prevent the City Engineer from requiring correction of errors or omissions in plans found to be in violation of any law or ordinance. _________________________ ________ CITY ENGINEER DATE 5. The following note shall be placed on all plan submittals: EXISTING CONDITIONS: The speed limit on _________________ is ________. The speed limit on _________________ is ________. The existing field conditions were verified during a site visit on ___________. 6. General 1) Signing and pavement marking design shall be shown in the same plan view. 2) Plan sheets are to be complete and to scale 1" = 40' unless otherwise approved by the City. 3) Entire length of project is to be shown in plan view. "Typical Sections" representative of striping and/or signing will not be accepted. 4) Signing and pavement marking plans shall include all existing signing and pavement markings for a minimum of 300 feet past the limits of construction and shall include adequate transitions and tapers to maintain traffic at the design speed. 6. Signing 1) All signs shall be graphically depicted, oriented for the plan reader. 2) All signs shall be stationed and referenced to the appropriate MUTCD sign designation with size noted. 3) Existing signs will be identified to either remain, be removed, or be relocated consistent with note 2 above. 4) Consultant shall field verify all existing advance or approach signing applicable to the project. Reference signs on plan sheet including location or station and note status of sign. 7. Striping 1) Existing striping shall be fully shown (as screened lines), identified by type and width, and completely dimensioned across roadway. 2) Raised pavement markers shall be graphically shown in plan view and referenced by construction notation. 3) All new striping shall be clearly identified noting color and line width. Include beginning and ending stations. 4) Striping to be removed shall be identified on the plans. 5) All pavement arrows, crosswalks, stop bars etc., shall be located by station or dimension lines. 8. General Reference Info 1) Pavement arrow markings in left turn lanes or deceleration lanes are located at the beginning of the pocket. 2) “Right Lane Must Turn Right” (R3-7R) 30”x30” signs are to be located at the beginning of the pocket adjacent to pavement arrow marking. 3) Bike lane striping will be 6” solid white line. Bike lane marking is the bike symbol only (no “ONLY” or arrow). Refer to MUTCD 2003 Page 9C-8 for the pavement symbol. For a 6’ bike lane, the pavement marking is 3’x6’. 4) Bike lane signage is the R3-17 sign. The sign and the pavement symbol will be placed together. 5) Right or left turn lane striping will be 8” solid white line. When required, type G RPMs are placed at 20’ spacing. 6) 4” solid white pavement edge lines are used on streets that do not have curb and gutter. 7) 4” broken white or yellow line should be accomplished by a 15’ stripe with a 25’ gap (40’ repeated interval). These lines must be drawn to scale on the plan set. 8) Stop bar and crosswalk will be 12” solid white 9) Cat tracks are used at intersections with dual turn lanes. These are 4” white stripes, with 2’ line segment and a 4’ gap. For drop lanes, cat tracks are an 8” white stripe, with a 3’ line segment and a 9’ gap. 10) Minimum size for R1-1 signs (STOP signs) is 30”. In the case of a wider roadway with a carrot, a 30” stop sign shall be used in the carrot and a 36” sign shall be placed on the shoulder. 11) Speed limit signs are generally placed every ½ mile for arterials. 12) FYI – Roadways will not be open for use until both the signing and the striping is complete.
