Registration
Information for Grades 1 - 5
Only a one-time registration fee is
required, there is no weekly fee to attend the program. The Summer
Recreation program is for school children enrolled in Grades 1 - 5 and is a
drop-in program. This means that the children may choose to sign
themselves in/out any time during program hours.
PARENTS: If you need day care supervision or extended
hours, please refer to the Summer Camp
Program.
Participants: Children entering grades 1-5

Dates: May 27—July 17 CLOSED Friday, July 4
Hours: Monday—Friday 8:00 a.m.—2:30
p.m.
On-site registration begins May 27
Fees: $50 Residents/per child

T-Shirt: Included (additional shirts are available for $10 each)

Peoria Unified School District will be offering free breakfast and lunch at
these sites. (Country Meadows - month on June only)
SCHOOLS NOT HOSTING
A GRADES 1-5 SITE WILL BE PROVIDED TRANSPORTATION TO THE FOLLOWING
LOCATIONS:
Sun Valley to Country Meadows; Parkridge
and Zuni Hills to Vistancia; Apache and
Frontier to Coyote Hills; Cheyenne, Paseo Verde and Desert Harbor to Skyview; Peoria, Santa Fe
Ira Murphy and Sundance to Oasis; Cotton
Boll to Alta Loma. No sites will bus to Oakwood. Participants must be in front of the school office by
7:40
a.m. to wait for the bus & will be returned by 2:30 p.m.
PROGRAM REMINDERS
- A one-time registration fee is charged.
- There is no weekly fee to attend.
- Optional field trips will be available at
an additional cost.
- Summer Recreation is a drop-in program.
Children may sign themselves in/out during program hours.
- Summer Recreation has a late pick-up
policy that is strictly enforced. Please make arrangements for your
child to get home before the sites close.
- Special needs:
If your child has a disability that would necessitate some assistance or
consideration, please call Kathleen at 773-7108 and ask for a Request for
Accommodation form. Upon completion, please attach to the registration
form and a staff member will contact you.
- All children registering for the Summer
Recreation Program must have the ability to independently feed, dress,
and toilet themselves. Exceptions would only apply if direct care staff
were funded through DES/DDD.
- The program provides a variety of on site
activities plus off-site trips for summer fun!!
- All participants must bring a
non-perishable lunch & drink daily.
- Snacks will be available for purchase
on-site.
- Drop-in format allows your child to attend
days that are convenient for you.
- Summer
Rec t-shirts help identify your child when attending off-site trips.
