Peoria businesses must file the monthly, quarterly, or annual Transaction Privilege (Sales)/Use Tax Return form.

When you are first issued a license, you will be required to report monthly. After at least six months of reporting, you can request to be changed to calendar annual or quarterly filing. The requirements for filing are:

· Annual filing: If annual taxable revenue is under $5,000
· Quarterly filing: If annual taxable revenue is between $5,000 & $50,000
· Monthly filing: If annual taxable revenue is over $50,000

The tax return is due on the 20th of the month following the reporting month.  Postmarks are NOT proof of timely filing and late fees will be assessed if received after the last day of the month.

The City mails the tax return to businesses that are not enrolled in eTax and it is greatly preferred that this form is completed and mailed to the City; this facilitates faster posting to the account and helps to ensure accuracy. 

However, should the mailed form not be available,  The Peoria Fillable Tax Return may be downloaded to your browser and then saved or printed, or can be saved directly to your computer's hard drive. The PDF Form Does NOT include the directions and codes. It will perform most of the calculations for you.  This PDF form must be filled out on your PC before printing.
